IMPORTANT HEALTH COVERAGE TAX DOCUMENT INFORMATION FOR 2025

Details: The 2025 Form 1095 documents are prepared and available upon request. Form 1095 illustrates information about offers of coverage made to full-time and part-time eligible employees as well as those who enrolled in Rogers Enterprises’ group health plan. To request or download your 1095 form, choose the method below that is suitable for you.

1. Active Employees: Download the 1095 Form on PlanSource

  • Follow the usual format for the username (first initial of first name, followed by first six letters of last name, then last four of social security number). If that login information does not work, contact Paula Wagener by phone at X 7533 or 708-679-7533.
  • Enter your password.
  • If you do not remember your password, click on “Need Help” under the login button.
  • After logging in to PlanSource, click on Documents on the left-hand, black shaded menu.
  • Next, select Tax Documents.
  • Finally, click on Current or Prior Calendar Years to view the 1095-C IRS form you need.
  • These steps will allow you to view your latest 1095-C form and any forms from prior years.

2. Active or Former Employees: Request to Receive the 1095 Form via Email

Email your request to [email protected].

  • The 1095 form will only include the last four digits of social security numbers pertaining to you and covered family members (if applicable). It does not include any other confidential data.
  • If you wish to receive the 1095 at a different email address than the one you use to contact us, indicate that in your message.

3. Active Employees: Request to Have the 1095 Form Sent to Your Store

You can request to have the 1095 form sent to the store you are assigned to, via interoffice mail.

4. Former Employees: Request by Phone or Mail

Call or mail your request for the 1095 form. If you call and request for the form to be mailed, you must verify your address before the form can be mailed to you.

Action to be Taken:

Review the options listed above if you need or want to receive your ACA 1095 form for the 2025 benefits/calendar year. Please note that the W-2 form is not the same as the 1095 form.
